A lot depends on the plant.

Use clean dirt in the new pot.
Once you get the plant out of the pot take the time to separate the roots.
Be sure to dump out any excess water so the roots do not become waterlogged.

Better perhaps if it can be done whilst dormant and the roots are asleep anyway.
